What is a One-Page Profile?

A One Page Profile is an amazing tool that communicates, in a simple and visual way, a positively focused summary of what others admire about your child, what is important to them, and how to best support them. The profile captures that important information about your child on a single sheet of paper. Without the need to read through a file, can help with any potential difficulties and avoid unnecessary meltdowns. It can help teachers, therapists, providers, caregivers, and others, provide person-centered care and support. It can help ensure that support is tailored to the needs, wants, and aspirations of the individual, rather than a one-size fits all approach.

Our template includes spaces to edit and share the following information about the individual: about me, I am working on, Things people appreciate about me, what is important to me, how to support me, and what doesn’t work for me.

The Kidz Club is a Medicaid Licensed Prescribed Pediatric Extended Care (PPEC), so there is no cost to families for children to attend. Our centers are staffed with a nurturing team of skilled nurses, state-registered nursing assistants, caregivers, and teachers. Physical, speech, and occupational therapists are on-site daily. Wheelchair accessible transportation on nurse-staffed buses is available to and from our centers.

Conditions that may require nursing interventions throughout the day include but are not limited to the following:
